He and his colleagues reviewed 25 years of medical malpractice claim payout data from the National Practitioner Data Bank and reported their findings in the journal BMJ Quality and Safety. Diagnostic errors were the most common source of the payments, the most expensive and the most likely to result in death (more so than surgical mistakes or medication overdoses).
